The parish of St Michael's is committed to the Safeguarding policies of the Catholic Church in England and Wales; to protect children and vulnerable adults from neglect, physical, emotional and sexual abuse and to promote a safe environment for them.

CHILDREN'S LITURGY PARENTAL CONSENT FORM

A Parental Consent Form should be signed by a parent, who wishes their child to attend the Children's Liturgy.
Please find the form below. Please complete a form for each child who wishes to attend. You only need to fill this
in once, not each time your child comes to Children's Liturgy.

Each week, you will need to register your child as you enter the church. The register will contain a list of children for
whom a Parental Consent Form has been completed. A parent/guardian will need to accompany the child to Children's Liturgy
